Skilled Nursing (Registered Nurse)

Initial Evaluation Visit
Reevaluates Patient’s Nursing Needs Including but not limited to Total Parenteral Nutrition (TPN)
Initiates the plan of Treatment and Revisions
Provides services Requiring Substantial Specialized Nursing Skills
Initiates Appropriate preventive and rehabilitative Nursing Procedures
Prepares Clinical and progress Notes,
Coordinates Services
Counsel the patient and Family
Participates in In-Service training programs and Supervises and teach other Nursing Personal.
  Home Health Aides
Performs total care or assists patient in all Activities of Daily Living / Personal care
Assists Patients with transfer , ambulation and exercises as assigned
Prepares Nutritious meals within the Patient’s diet and assists patient with eating when necessary
Performs Light Housekeeping Chores, which facilitates patient-self care in the home setting
Assists Patients to the bathroom or with use of the bedpan
Answers Clients' / Patients' calls and attend to their request promptly
Meet the safety Needs of the patient and Uses Equipment Safely and properly (e.g. bedrails and Assistive Device)
Provides proper care and Observation of the patient’s skin to prevent breakdown of tissue over bony prominence.
Observes general Conditions and report changes to the Supervisor.
Completes Records accurately and Carries Out All Assignments as required.

Depending on Availability, the Home Health Aide is assigned to a particular patient /Client by a Care Manager.
