Prepares Nutritious meals within the Patient’s diet and assists patient with eating when necessary |

Performs Light Housekeeping Chores, which facilitates patient-self care in the home setting |

Assists Patients to the bathroom or with use of the bedpan |
|
Answers Clients' / Patients' calls and attend to their request promptly |
|
Meet the safety Needs of the patient and Uses Equipment Safely and properly (e.g. bedrails and Assistive Device) |
|
Provides proper care and Observation of the patient’s skin to prevent breakdown of tissue over bony prominence. |
|
Observes general Conditions and report changes to the Supervisor. |
|
Completes Records accurately and Carries Out All Assignments as required. |
Depending on Availability, the Home Health Aide is assigned to a particular patient /Client by a Care Manager.
